Heavenly Med Spa & IV Lounge LLC Patient Policies

  • We ask that you arrive at least 5 minutes prior to your scheduled appointment. This will allow ample time to complete a client intake and the necessary consent forms if needed. We always leave the option open for our patients to fill out their paperwork prior to arriving at their scheduled appointment via email.

  • Children under the age of 10 are not permitted in the suite at anytime. This is for both the client and child’s safety. If you show up to your appointment with a child, your appointment will need to be rescheduled and you will be charged the no show/late cancellation fee.

  • Scheduled treatments are reserved especially for you. We require a 48-hour advance notice in order to cancel any service at no charge. Any changes or cancellations made by the patient after the 48-hour window will result in a $100 fee. If you cancel less than 48 hours you will be charged $100! If there’s no card on file you account will have a $100 fee added. If you are a NO SHOW you will be charged half of the service fee or $200 (for neurotoxin)

  • If you are running late for your appointment, please call 931-377-9210 and inform us of your ETA. All appointments have been designed to allow appropriate time for each service. If arrival is delayed, we will make every effort to accommodate your full appointment, but this is not guaranteed. Service time may be abbreviated to avoid delays for other patients. Abbreviated appointments could mean less numbing time for treatments that require it or the omission of additional services you were scheduled for that day. Appointments missed by 10 minutes or more will be considered a No Show. After two (2) no shows, you will not be scheduled to be seen again.

  • Products purchased at the office are not eligible for return or exchange. Products bought online may only be exchanged for the same product if damaged during shipping.

  • We do not offer refunds on services rendered even if you are disappointed in the result or unhappy with the outcome. As stated in our consent forms, “I understand that the results of my treatment cannot be guaranteed. I understand that my results may vary based on the following factors: skin type, area of body being treated, post treatment care. I understand I may require multiple treatments to obtain the desired results.” It is up to the discretion of our patients to move forward with treatment after the consent form is signed and before said service is rendered.

  • We are committed to continuously expanding our services to ensure that we bring you the latest and greatest technology and treatment techniques. Although we make every effort to keep our website and service menu updated accordingly, please note that prices, services, and products are subject to change at any time without notice. Our office works in conjunction with applicable rewards programs (i.e., Aspire,Evolus, and Alle) to obtain reward points earned on previous injection services rendered or gift cards that were purchased through the manufacture’s website. It is the responsibility of the patient to notify us points/gift cards are being used prior to checking out.

  • We love having you as our patient, but Heavenly Med Spa & IV Lounge LLC reserves the right to refuse service to anyone, at any time, for any reason.

  • All major credit cards are accepted. Any gift cards, coupons or treatment packages must be mentioned prior to checking out.

    A 3% credit card fee will be added at all credit card charges.

  • Due to the nature of our business, we are unable to work with your insurance company to gain coverage for the services we provide. These services are considered to be “out of pocket” and 100% the patient’s responsibility. We do accept HSA cards as long as a VISA is present on the card.

  • Our gift cards can be purchased at our office or over the phone. Gift cards are not returnable and are redeemable for services or retail. They are not redeemable for cash or for the purchase of other gift cards. Heavenly Med Spa & IV Lounge LLC is not responsible for lost or damaged gift cards. The value of the gift card may be exchanged for services and/or products. Gift cards are not valid for gratuities.

  • Gratuities are not accepted at Heavenly Med Spa & IV Lounge LLC. The services we provide are a luxury and therefore will not be accepted.

  • First and foremost, our goal is for you to be happy with your service and to love the results you achieve. We believe in full disclosure, which means we strive to ensure that you understand the treatments you receive, and the risks and benefits associated with each of them. Please be assured that our team will only recommend treatments and products that will benefit you specifically and which are medically appropriate for you, and you only. We are dedicated to our patients and ask that if you have any questions, please contact us at any time.

  • Touch up after tox treatment is to be scheduled as a "follow up" appointment, either done after treatment or may be done by booking online. Touch up are only done between week 2-4. Patients will not receive a complementary touch up prior to 3 weeks (21 days) and after 4 weeks (28 days). It will be the responsibility of the client to pay for any necessary touch ups. There are no exceptions.
